Insanity is a game mechanic mainly present in The Depths, but also on the Surface to a lesser extent.

Insanity is dictated by a meter that is not directly shown to the player, but its progress can be tracked by a blue hue on the border of the screen. Once your sanity has decreased enough, it will begin to debilitate your character in increasingly harsher ways the more insane you become.

Players start with 80 base sanity. Being in The Depths passively takes sanity at a rate of 1 point every 30 seconds, meaning you can spend about 40 minutes in The Depths with 0 Willpower and no equipment bonuses before you reach maximum insanity and proceed to inevitably die

Insanity can be resisted by leveling up the Willpower attribute, by taking Talent cards from the Mental Fortress deck, wearing a blindfold, or wearing equipment with a sanity bonus.

When losing enough sanity, the player will endure negative effects.

Panicking Stuns you for 1 second and deal 10 damage (5 with Grasp on Reality Talent). Is then followed by shivering. With the Unfazed Talent, this effect is ignored and replaced by 2.5 seconds of shivering. <40% 30–36 seconds
Hidden Faces When panicking or scratching, hide the face and name of nearby humanoids (including players) for 30 seconds. <40% 360 seconds
Scratching Stuns you for 1.5 second and deal 20 damage (10 with Grasp on Reality Talent). Is then followed by shivering. The Unfazed Talent offers no protection against this. <10% 30–36 seconds
Execution If knocked by any means, including scratching, you are immediately killed. <10% N/A
Head Explosion Reaching 0 Sanity with a No-Life King enchantment in The Depths or while observing the eyeballs in The Ancient Rotlands will immediately kill the player. 0% N/A

Sanity Decreasing Effects[]

  • Being in range of the Megalodaunt or Primadon's screech attack.
  • Being hit by the Deep Widow's web or bite attack.
  • Being hit by the Mudskipper Broodlord.
  • Being in the First Layer.
  • Being hit by a player with Shared Misery active.
  • Being hit by the Ardour scream of a player with the Echoing Lunatic talent.
  • Drinking or being splashed by a Mindbreaking potion.
  • Using Heretic's Sutra.
  • Using Blindseer Mantras.
  • As a potential corrupted resonance downside; gives a massive amount of insanity, making it dangerous to use in general.
  • Being hit by a weapon enchanted with Curse of the No Life King, which gives you a 2% sanity decrease.
  • Equipping a weapon enchanted with Curse of the No Life King, which constantly gives you insanity when unsheathed while in The Depths.
    • The effects of this can be reduced through increasing Willpower. However, you'll still die if you don't regenerate your sanity.
  • Being in the immediate vicinity of the pulsating brains with eyes and looking at them in Ancient Rotlands.
  • Having the Wishbound flaw during nighttime.

Sanity Restoring Effects[]

  • Passive sanity regeneration. Regen 0.25 sanity per second at day time and 0.05 at night time. (Disabled in the Depths)
    • The Breathing Exercise grants 50% more passive sanity regeneration, granting 0.375 sanity during the day, and 0.075 sanity at night.
  • Sitting at a campfire (Improved if sharing the campfire with a Capra) (Graceful Flame included) regenerates 0.5 sanity per second.
  • Receiving the Mark of Jurik from a Capra.
  • Drinking or being splashed by a Heartening potion.
  • The Conquer Your Fears Talent grants sanity when depths mobs die or when allies grip humanoid enemies near you.
  • The Castle Light and Chariot's Spire sanity aura, which are exclusive to the Deepbound and Ignition Delver Origins respectively.

Trivia[]

  • While often attributed to him by the community, Yun'Shul is not the cause of Insanity; rather, the eldritch nature of the First Layer is responsible. This is likely why the Second Layer does not cause Insanity, as despite its variety of monsters it's more grounded in reality.
  • With just sanity increases (blindfold, Mental Fortress Talent deck, 100 willpower and Blindseer Oath) and no randomly rolled sanity stat on gear and no potions, players can reach 436 sanity, which equals to just below 2 hours (118 minutes) in The Depths before tier 3 insanity.
  • Each point of sanity equals to 30 extra seconds in the Depths before reaching 100% insanity.
  • The spoken phrases caused by Heretic's Sutra consist of the following (albeit without the same degree of distortion):
