The Monkey's Paw, as the name suggests, is an island shaped like the paw of a monkey. It has a large beach, with a ship post and many palm trees. Nearer the center of the island is an opening in the rock, with leaves falling from the top, similar to the Deep Owl's feathers. This leads to the main area of the island.
Once you enter the main area of the island, there is a ridge which overlooks a massive flat ground. To the left and to the right of the ridge, there's 2 ropes that lead down into it.
The island is very abundant with food. Calabash, wheat, mushrooms, and other things can be found at this island. If you're a carnivore, there's not much for you to eat besides the players who come here to fight Primadon or the Elder Primadon.
On the edge of the ridge there's a fire that is always burning. You can interact with the fire and make a sacrifice. You can sacrifice food, mantra modifiers, and raw materials. This fire is used to summon the Primadon, a massive aquatic ape, or the Elder Primadon, a stronger variant. The recognized items that work to summon Primadon are three Umbral Obsidian or five Megalodaunt hides and for Elder Primadon it's four Sinner's Ashes or ten Idols of Yunshul. Once the Primadon is successfully summoned, the island's name changes to the Primadon's Hunting Grounds.

- The music for this area when fighting Primadon and the Elder Primadon is A Symphony Of Strife by Naktigonis
- The location's name is likely a reference to the story "The Monkey's Paw" by W.W. Jacobs, where a family of three receives a severed monkey's paw that will grant them three wishes. However, each wish comes with a terrible consequence.
- The wish in this game is the items used in the Sacrificial Flame, and the "consequence" is the Primadon coming up from out of the water.
- Additionally, the island tells you to "Be careful what you wish for." which is the central idea of the story.
- Be careful while you are here, as this is a popular ganking spot for many players and you may stumble upon people which will attack you after you kill Primadon or even during it, exercise caution and make sure no undesirable third party is waiting to ambush you here before starting Primadon's boss fight.
- The Monkey's Paw, along with Layer 2 floors 1 and 2, are the only few places where Roblox terrain is used for its environment.
